Nominations forms for 2010 are due by February 18, 2010 and should
be sent to the DTVY Chairperson:

in the December Newsletter or is available in 3 different formats on
this web site:
- DTVYForm.pdf -
This is the best choice if you want to print it and then fill it
out in long hand. You will then need to mail it to the
DTVY Award Chairperson via
U.S. Mail.
- DTVYForm.doc -
This is the best choice if you have a word processor and want to
key your information into the word processor. You will
then need to mail it to the DTVY Award Chairperson either as an attachment to an email,
or via U.S. Mail.
- DTVYForm.htm -
This is the best choice if you want to key the information
directly into the web page as displayed by your web browser.
You will then need to print the form and mail it to the DTVY
Award Chairperson via U.S.
Mail.
